Payment is due at the time of your appointment

If you need to cancel your appointment, please give us 24 hours notice

For your first visit, please remember to bring your insurance card as well as an ID and any paperwork necessary for the physician 

If you do not show up or cancel your appointment within 24 hours, you will be charged a fee of $25
